Our Appointment Process

Taylor Street Primary Care Clinic is a full-service primary care clinic that sees patients of all ages. Same-day appointments may be available for acute illnesses or recent injuries. Routine health concerns require a scheduled appointment.

STOP IN

Feel free to stop by the clinic at 1550 Taylor Street to schedule an appointment. We may be able to accommodate you later the same day, but if not, we will schedule an appointment in the next few days when it is convenient for you.

PREPARE

Fill out the necessary registration forms and bring them with you to your appointment. Make sure you have updated insurance information. If you do not have insurance, we may be able to assist you with resources so that you can obtain insurance.

 

ARRIVE

Plan to arrive fifteen minutes prior to your appointment. Bring a government ID (driver’s license, state identification, or passport) and your insurance card (or a copy [front and back]).

Please call the Taylor Street Primary Care Clinic promptly if you are unable to come an appointment.  We ask that you call at least one hour prior to your appointment time. Appointments are in high demand, and your early cancellation may provide another person access to timely medical care.

To cancel or reschedule an appointment, please call 313-486-5501. Please keep in mind that our operating hours are between 9:00am and 5:00pm Monday through Friday.

A “no show” is someone who misses an appointment without calling to cancel or reschedule their appointment. This also includes patients who are not present within ten minutes of their appointment time.

Any appointments that are cancelled without a one-hour notice are considered “late cancellations.” Late cancellations will be treated as a no show.

Enrollment Form

Be sure to bring your completed form with you to your first appointment.

Sliding Scale Form

Discounts are offered based upon household income and size. A sliding fee schedule is used to calculate the basic discount and is updated each year using the federal poverty guidelines. Once approved, the discount will be honored for one year, after which the patient must reapply.
